Position Summary:

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Hours / Schedule: PRN; as needed

Minimum Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program
  • LPN/LVN licensure in state of practice
  • Prior home health or hospice experience preferred w/ minimum of 1 year medical/surgical experience
  • Current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
  • Demonstrates excellent observation, problem solving, verbal and written communications, skills per competency checklist
  • Shows ability to organize and prioritize workload independently

Key Responsibilities:

  • Provides direct patient care within the established Plan of Care and as defined in the State Nurse Practice Act.
  • Demonstrates exceptional service to patients, families, referral sources, co-workers and other customers. Assists team members as needed to ensure that this level of service is consistently provided.
  • Implements planed interventions to meet the physical, psychological, social and environmental needs of the patient.
  • Evaluates the patient's responses to the approved plan of treatment.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of hospice services, appropriately reporting any identified needs to the interdisciplinary group.
  • Participates in Continuous Care as requested.
  • Participates in clinical on-call program.
  • Attends Interdisciplinary Group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
  • Performs other duties as assigned.
