Are you a talented Psychiatric Mental Health Nurse Practitioner or Family Nurse Practitioner with a specialty/experience in Psychiatry? We are looking for an NP to care for our patients in the outpatient clinic setting!
Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, and outpatient care. As we continue to expand our services & locations, our community has grown far beyond Paris. We have cutting edge technology, new offerings for the communities we serve, strong specialty support, and still have the small-town approach and values that make our organization so special. The Family Medicine Group is currently staffed with 14 physicians and providers, with specialist support in behavioral health, women’s services, general surgery, orthopedics, pain, dermatology, podiatry, and cardiology. Additionally, we have 16 visiting specialists that see patients in the clinic each month. The hospital has a 24/7 hospitalist service and convenient care for patients. We have a physician friendly EMR. In 2022, we completed an expansion project adding 32,000 square feet to our Paris Clinic.
Essential Functions:
- Speaking with patients to better understand their concerns, needs, conditions, and medical histories.
- Performing examinations, ordering and interpreting diagnostic tests, and going over results with patients, if necessary.
- Establishing care or condition management plans, including prescribing medications or other treatments.
- Keeping records regarding patient history, condition, treatments, and progress.
- Practice according to Joint Commission Standards and IDPH regulations
- Utilize evidence-based guidelines to practice
- Work in collaboration with other members of the healthcare team and corporate team to provide quality service to patients
- Work to meet quality measures set forth by the facility
- Ensure company’s objectives regarding financials, patient engagement, and healthcare quality

Position Requirements:
- Master’s degree or Doctorate and a state license with the Illinois IDFPR, or willingness to obtain in the State of IL
- Excellent verbal and written communication, analytical, interpersonal, and active listening skills
- Passionate and committed to providing high quality, sympathetic care, and service to patients
